A Grace to Heal Counseling, we want you to understand your rights as a therapy client in Texas. This page outlines how you can access your records, contact regulatory agencies, and file complaints if needed.

Requesting Your Health Care Records

You have the right to request copies of your health care records at any time.

How to request your records:

• Email Adriana Lewin at: adriana@gracetohealcounseling.com

• Include your full name and date of birth in your request

• Specify which records you need (session notes, treatment plans, etc.)

Timeline: You will receive your records within 15 business days of your request.

Fees:
Billed at 15 minute increments based on the hourly rate listed in your consent form (as the rate increases annually)
